Coroner: Glee actor died from mix of heroin use and alcohol

Authorities in Canada released their final report on the death of Glee star Cory Monteith. The coroner's report finds the 31-year-old died from a mix of intravenous heroin use combined with alcohol.

Monteith was found dead in a Vancouver hotel room in British Columbia, Canada on July 13th. He had a history of substance abuse.

Wednesday's report ruled his death an accident.
