Investigators are still trying to figure out how a woman fell off a roller coaster at a Texas Six Flags last week. 52 year-old Rosy Esparza fell 75 feet and died. Records show the ride passed inspection last February. In Texas, the Department of Insurance inspects rides annually and parks must self report incidents. Since 2008, Six Flags reported 14 injuries involving the Texas Giant . Critics think safety loopholes in the amusement park industry puts riders at risk, and they're calling for tougher federal regulations.
